History

Synergy Medical Group was founded by physicians who recognized a critical gap in the patient journey after hospitalization.

Too often, patients were discharged without the continuity, follow-through, or specialty support needed to carry their care forward. Referrals went uncompleted, treatment plans became fragmented, and the transition between hospital and post-acute care left too much room for patients to fall through the cracks.

Synergy Medical Group was created to change that—bringing primary care physicians, specialists, and surgeons together through a coordinated, physician-driven model designed to keep patients connected to the care they need.
